There is a toll free phone number on back of your insurance card. Please call your insurance company to find out if your plan includes acupuncture benefits and ask the following questions:
1. Does my plan include acupuncture coverage? What are the benefits for acupuncture services? What medical conditions can be covered?
2. Does my plan covers covered both in and out of network? What percentage coverage for both situations?
3. What is my annual deductible if there is any? Is my deductible met currently? What percentage is reimbursed after the deductible is met? How much is the co-pay if there is any?
4. Is there an annual limit on my acupuncture benefits? How many visits a year allowed?
5. Is medical authorization required for my acupuncture treatment?
If you do not have insurance coverage, that is fine too. We have discounted prepaid pay plan if you pay at the time of the service. Just give us a call to find out. Do not forget to check out monthly specials.

Cancellation Policy: 24-hour cancellation policy applies. No show or same day cancellation will be charged $50. All checks returned by the bank for NSF will result in $35.00 fee.
